Project Recovery Intake Check-In Checklist
What to Bring :
Welcome to Project Recovery!
To help make your intake process smooth and ensure your companion receives the best possible care, please bring the following items with you.
Required Documents
☐ One (1) Valid Government-Issued Photo ID

☐ Admission Paperwork
Documentation from your cooperating:

Medical Detox Facility or
-
Intensive Inpatient Treatment Program
This paperwork helps verify your eligibility for Project Recovery boarding assistance, if you don't have this paperwork yet, we will help you get copies.
☐ Vaccination Records (If Available)
Please bring any veterinary records you have, including:

Rabies
-
Distemper (DHPP)

Bordetella (Kennel Cough)

Don't worry if you don't have copies. Our team will work with your veterinarian to obtain records whenever possible. If vaccinations are needed, Project Recovery will assist based on eligibility and available funding.
Your Companion
Please bring your companion to check-in on your scheduled intake date and time.
To help them feel comfortable during their stay, we encourage you to bring familiar items, including:

☐ Favorite blanket or bedding
☐ Favorite toy(s)
☐ Leash and collar/harness
☐ Medications (if applicable)
☐ Written medication instructions
☐ Any special feeding or care instructions
Don't Have These Items?
That's okay!
We understand that many participants are entering treatment during a time of crisis.
If you do not have food, bedding, bowls, toys, or other basic supplies, Project Recovery and Serenity Paws will gladly provide them whenever possible so your companion remains safe, comfortable, and well cared for during their stay.
Before You Arrive
Please remember to:
☐ Call ahead if your arrival time changes- if you need a ride, this is an extremely important step so that we may arrange transportation.
☐ Inform Project Recovery of any medical or behavioral concerns regarding your companion.
☐ Ensure your emergency contact information is current.
☐ Provide any medications your companion is currently taking.
